For the 2022-2023 School Year
All student health examinations must be documented on the NYS Required Health Examination Form or an electronic health record (EHR) compatible form.
*Please note the school year begins on July 1, 2021
If the physical was performed July 1, 2021 it is not valid and an updated physical must be requested.
